改名字要傳的資料 - 英檢

我的情況則是姓名的拼寫法和護照上不同，
於是寫信到ETS去請求改名協助，
他要求我傳真這一封回信、上述資料和護照影本回去，
我的做法是把護照掃瞄做成pdf檔當成附件，直接以e-mail回復他，
隔天就改好啦。

信件內容如下：
Dear Sir/Madam,

The data you request as fallow:
correct name:
address:
test date:
registration number:
ETS ID number:

The copy of passport is attached.

Please kindly correct my name.
Thank you very much.
