請問Should當句頭的用法.... - 自學

這是我不懂的一段句子,請教各位大大哦!
--> Should you not wish to have the kit sent,check the appropriate box
and postmark it no later than April 30.

不好意思,這個句子讓我卡很久
想要問的是:
(1)should放在句頭是什麼用法? 什麼時候會用到呢?
(2)should在這個句子裡是什麼意思?
(3)為什麼到kit sent的後面不是用問號(?) ?
(4)為什麼要用have the kit sent這種倒裝句型?
是因為should放句頭的關係ㄇ?! 為什麼不能直接用send the kit ?
